What is Endoscopic Sinus Surgery?

I think it will be useful to talk about what sinusitis is.  Sinuses are the gaps in facial bones protecting our brain against traumas arising from sudden impacts. These gaps are open to nose. Sinusitis is the infection of these passages between nose and sinus.

Why is sinusitis surgery is performed?

Acute sinusitis is the type responding to therapy and disappearing in 2-3 weeks. However, if the disease resists to therapy, it is called as chronic sinusitis, then. In a person with normal anatomy, sinusitis usually presents with an acute course, usually with antibiotic and drainage treatment, but in people with some anatomic variation, in people with nasal polyps, or in fungal infections, sinusitis does not disappear without our external intervention. Thus, we intervene to patients with surgery and eliminate sinusitis.

This anatomic variation may be septum deviation blocking nostrils, naturally narrow sinus opening, wide cells blocking sinus opening or nasal polyps or bulks. The main aim is eliminating the blockage in sinus openings and discharges the infection.

What is nasal polyp?

Nasal polyps are pale pink growths arising from nasal and sinus mucosa and filling sinus gaps. They are not malignant. People mostly confuse nasal polyps with concha hypertrophy making them feel concha grows, however, they are not the same. Incidence of nasal polyps is about 1,4% in adults and 0,01% in children. Nasal polyps generally show no indications as long as not blocking sinus openings or growing significantly. However, in the case that they block sinus openings, mucus in sinuses cannot circulate and may cause infection and cyst formation

Moreover, when overgrown polyps block nasal cavities, it may cause problems such as nasal blockage, olfactory disorders, snoring caused by mouth breathing, mouth dryness, throat burning and postnasal drip Patients with aspirin sensitivity are tend to develop nasal polyps. I say to my patients with aspirin sensitivity and applying for nasal polyp operation this certainly: “Because of an unknown mechanism, your nose will continue to develop polyps after the surgery. You will most probably need for another surgery in a few years.” This is an unwanted situation, however, another solution has not been founded yet. This situation is valid only for patients with aspirin sensitivity and nasal polyps.

How long does it take for a sinusitis patient to recover?

I generally discharge my patients on the same day upon this operation, however, I sometimes make them to stay for 1 post-operative day depending on her/his situation.  I generally prefer tampons without airways after the operation. Because blood leakage from nose continues for 1-2 post-operative days. Tampons with airways annoy my patients greatly. Thus, I use tampons without airways for 2 or 3 post-operative days for comfort of my patients.

I remove these tampons on the third or fourth post-operative day and suggest them to avoid from heavy work and wash their noses with sf and use antibiotics and painkillers. The patients generally can get back to their works in 5-6 days.

How is sinusitis surgery is performed?

Sinusitis surgery is carried out via rigid endoscopes inserted in nostrils without any cuts. It lasts about 1-5 – 2,5 hours.
